Data Privacy Agreement

PHINMA Education Holdings, Inc. (PEHI) recognizes its responsibilities under the Republic Act No. 10173 (RA 10173), also known as the Data Privacy Act of 2012, with respect to the data they collect, record, organize, update, use, consolidate or destruct from the data subject. The personal data obtained from this portal is entered and stored within the college authorized information, enrollment and communication system and will only be accessed by the PEHI authorized personnel. The PEHI had instituted appropriate organizational, technical and physical security measures to ensure the protection of the Students personal data. Furthermore, the information collected and stored in the portal shall only be used for the following purposes:

1. Processing and reporting of documents related to the enrollment, CHED, DepEd and other accrediting agencies under certain conditions as required by law.
2. Announcements/ promotions of events, programs, courses and other activities offered/ organized by the College and its partners.
3. Activities pertaining to establishing relations with students. PEHI shall not disclose my personal information without my consent and shall retain my information over a period of ten years for the effective implementation and management of information.

Outside of your regular class hours, how often did you study, review, or do schoolwork on your own during Senior High School? *

Rarely or never: I mostly just listened in class and only studied right before major exams. Occasionally: I only studied for a few hours if a heavy assignment was due that week. A few times a week: I regularly set aside an hour or two every few days to keep up. Almost daily: I reviewed or read almost every evening after school.

Senior year can get incredibly overwhelming when deadlines pile up and requirements get heavy. During your grade 12 year, how often did you find yourself missing school days, cutting classes, arriving late, or submitting your projects past the deadline? *

Honestly, this happened quite often (almost every week I had backlogs, lates, or absences). It happened sometimes (maybe once or twice a month when school requirements got too busy). It rarely happened (only a few times across the entire school year). I managed to avoid this completely and always attended school and submitted everything on time.

When you ran into a very difficult lesson or a confusing assignment in high school, what was your most common way of handling it? *

Figure it out alone: I preferred to look for answers online (YouTube, Google) or read my notes instead of asking people. Ask classmates: I usually asked my friends, classmates, or seatmates for help first. Ask teachers: I proactively approached my teacher or subject adviser during breaks or at the faculty room. Let it pass: I usually just let it pass, skipped the hard parts, and hoped for the best.

How often did you talk to a high school teacher or section adviser about your personal goals, college plans, or career choices? *

Regularly: I looked up to my teachers as mentors and frequently asked them for life or career advice. Occasionally: We chatted about my future plans or college choices a few times a year. Rarely: I only talked to them if it was directly about a grade or an assignment requirement. Never: I preferred keeping a clear boundary between myself and my teachers.

When preparing for big exams or major school requirements, which study setup did you use most of the time? *

Alone: I almost always studied entirely alone because studying with a group easily distracts me. Occasional groups: I occasionally joined informal group reviews with classmates right before a major test. Regular groups: I consistently reviewed lessons with a group of friends (either in person or via group chat). No extra study: I didn't really do much extra studying outside of class, whether alone or with a group.

When teachers were discussing heavy subjects (like Math or Science) in your regular high school classroom, how did you handle the lessons? *

Easy for me: I understood the lessons quickly and kept up with the best students without much effort. Just right: I was right in the middle, keeping up with most of my classmates at a normal pace. Hard for me: The lessons often felt too fast or difficult, and I had to spend a lot of extra time and effort to catch up. I didn't mind it: I didn't really pay attention to my lessons or care about how I was doing compared to others.
